Communities in Action

The CHAP program is designed to serve as a model in health service intervention for underserved residents of the state. A partnership was formed between the NC Office of Minority Health & Health Disparities and community and faith-based organizations to work together in an effort to “close the gaps” to health care in underserved communities while reaching out to replicate the process across the state. Success Dynamics CDC provided the leadership in developing partnerships among more than 25 faith-based and community organizations, Community Empowerment Network (CEN) for the purpose of organizing and building capacity to provide health services to underserved residents of the state. CEN’s mission is to promote social (networking), economic, health, education, and community development through the partnerships within target counties across the state of Georgia.
